Your Privacy Rights

  • You have the right to access your own health record.
  • You may request corrections if you believe information in your record is inaccurate or incomplete.
  • You may withdraw or limit your consent for sharing your information, except where disclosure is required by law.

How We Protect Your Information

  • Records are stored securely and accessed only by authorized staff.
  • Electronic records are password-protected and encrypted where possible.
  • Paper files are kept in locked storage.

How We Use Your Information

  • To provide you with safe, coordinated care.
  • To meet legal and regulatory requirements.
  • To support program planning, evaluation, and quality improvement (always in de-identified form).

Sharing Your Information

  • Information is shared only with members of your care team, unless you provide consent for other disclosures.
  • We do not sell or use your personal health information for marketing.
  • In rare cases, information may be shared without consent if required by law (for example, if there is a risk of serious harm or mandatory reporting obligations).

Retention and Disposal

  • Once the required retention period has passed, records are securely destroyed.
  • Health records are kept in accordance with provincial legislation.
