Please note: we cannot respond with medical advice or set-up appointments on your behalf. This email is not confidential. Please do not disclose medical or personal information. Additionally, this email may not be used for ongoing care. Finally, the Health Centre is decentralized across four sites. We cannot respond to questions about the immediate whereabouts or availability of Health Centre staff. As always, you can check in with the Systems Navigator.
Fax Your Medical Records
Out of respect for client privacy, please do not send any medical documentation to our general email. Medical records must be sent by fax to (519) 642-1532. Medical records sent to our general email will be destroyed.
