Seniors’ Wrap Around

The objective of WrapAround program is to support senior individuals age 55 plus to live independently and integrated in the community. The program targets not only health concerns but also all aspects that could affect the life of a senior person (emotional support, financial, social, family support, spirituality, housing, educational, among others).

Seniors French Health Promotion Program

The program is designed for French-speaking individuals, especially adults and seniors aged 50 and older, who face difficulties accessing health information or navigating the healthcare system, and who encounter challenges related to language, health conditions, or any other barriers affecting their health and well-being.

Primary Care

Our Primary Care team works together to keep you as healthy as possible. Care is provided by registered nurses, registered practical nurses, nurse practitioners, social workers, physicians, a community dietitian, and physiotherapists. Together, they provide treatment, health teaching, and support to help you reach your health goals.

Systems Navigation & Intake Coordination 

Learning about all the programs and services at the Health Centre can feel overwhelming. Our Systems Navigators are here to help. They listen to each person’s story, assess their unique situation, and connect them with the supports that best meet their needs.

Gender Care

The Gender Care Program provides gender-affirming medical and social supports through a wraparound team that includes nurse practitioner, a registered program nurse, social worker, a community health worker, and links to affirming specialists.

Team Based Care

Through Team Care, eligible patients can access a wide range of services – including mental health supports, physiotherapy, system navigation, housing support, and more – at no cost. The program helps bridge gaps in care for individuals with limited financial resources and complex health or social needs.

MyCare – HIV Treatment Program

The MyCare HIV Treatment Program is a collaborative partnership between the London InterCommunity Health Centre and St. Joseph’s Infectious Diseases Care Program (IDCP). It provides comprehensive HIV treatment and support for individuals who face barriers to accessing traditional, hospital-based care.

Hepatitis C Care Team 

The Hepatitis C Care Team is a partnership between the London InterCommunity Health Centre and Regional HIV/AIDS Connection, offering free hepatitis C testing, treatment, and support for individuals at all stages of the disease.

Options Clinic – Anonymous HIV Testing 

The Options Clinic offers free, rapid, and anonymous HIV testing to individuals at increased risk of HIV. Our aim is to make HIV testing accessible and stigma-free for those who might otherwise delay or avoid it.

Health Outreach Mobile Engagement

The H.O.M.E. Program brings health and social services directly to individuals who are unhoused, insecurely housed, or newly at risk of homelessness. Services are provided in shelters, encampments, and other locations across the city.
