We provide ID services to individuals experiencing homelessness. Clients may be asked to bring OW/ODSP statements to qualify. 

What We Offer 

  • Health Cards: Form provided to take to Service Ontario (100 Dundas St.). A temporary card may be issued same-day; a permanent one arrives in 2–4 weeks. 
  • Birth Certificates: Our Same Day Community Workers can process all types of birth certificate applications. 
    • Ontario: ~8–10 weeks 
    • Out-of-province: ~12–16 weeks 
  • Permanent Resident Cards – by appointment only 
  • Citizenship Cards – by appointment only 
  • Affidavits – by appointment only (SIN required) 

Please Note: We do not assist with: 

  • Passports (Cherryhill Mall, 301 Oxford St.) 
  • Ontario Photo ID Cards (Service Ontario) 
  • Status Cards (contact your Band Office) 
  • SIN Numbers (Service Canada, 457 Richmond St.) 
  • Driver’s Licences (Service Ontario)
