Our staff meet people where they’re at in life – no matter their current circumstances. We work within a health promotion framework grounded in inclusiveness, relationship-building, and harm reduction. 

How We Can Help 

The Health Outreach team provides support with: 

  • Mail services 
  • Addiction assessments and referrals to treatment 
  • Harm reduction education and supplies 
  • Group activities 
  • Housing referrals and loss prevention (for clients registered with our primary care team) 
  • ID services 
  • Ontario Works and ODSP applications (for clients registered with our primary care team)
