The Health Outreach team provides support with:
- Mail services
- Addiction assessments and referrals to treatment
- Harm reduction education and supplies
- Group activities
- Housing referrals and loss prevention (for clients registered with our primary care team)
- ID services
- Ontario Works and ODSP applications (for clients registered with our primary care team)
