- Records are stored securely and accessed only by authorized staff.
- Electronic records are password-protected and encrypted where possible.
- Paper files are kept in locked storage.
How We Use Your Information
- To provide you with safe, coordinated care.
- To meet legal and regulatory requirements.
- To support program planning, evaluation, and quality improvement (always in de-identified form).
Sharing Your Information
- Information is shared only with members of your care team, unless you provide consent for other disclosures.
- We do not sell or use your personal health information for marketing.
- In rare cases, information may be shared without consent if required by law (for example, if there is a risk of serious harm or mandatory reporting obligations).
Retention and Disposal
- Once the required retention period has passed, records are securely destroyed.
- Health records are kept in accordance with provincial legislation.
