The MyCare HIV Treatment Program is a collaborative partnership between the London InterCommunity Health Centre and St. Joseph’s Infectious Diseases Care Program (IDCP). It provides comprehensive HIV treatment and support for individuals who face barriers to accessing traditional, hospital-based care

MyCare supports people who are: 

  • Living with HIV 
  • Experiencing homelessness or unstable housing 
  • Living with mental health and/or addiction challenges 
  • Incarcerated or recently released 
  • Facing stigma or system-related barriers to care 

How the Program Works 

Many MyCareclients are rostered through Health Outreach at the Health Centre and work with an interdisciplinary team using a case management model. The program connects clients to: 

  • Full-time nursing and outreach support 
  • Infectious disease specialists (on-site clinics every 6–8 weeks) 
  • Medication adherence support (including daily outreach for medication administration when needed) 
  • Wraparound services for physical, emotional, and social health 

Where Care Happens 

Care is delivered where clients feel most comfortable. This includes: 

  • 659 Dundas Street (Health Centre satellite HIV clinic) 
  • The client’s home 
  • The street 
  • Emergency shelters 
  • Outreach locations 

Undetectable = Untransmittable (U=U) 

People living with HIV who take antiretroviral therapy (ART) and maintain an undetectable viral load cannot sexually transmit the virus to others. This concept, known as U=U, is backed by strong global scientific evidence and promotes both personal and public health. 

Maintaining an undetectable viral load requires: 

  • Proper treatment selection 
  • High levels of medication adherence 
  • Regular monitoring 

When viral load is suppressed, HIV becomes untransmittable through sex—an empowering fact for individuals living with HIV and their partners. 

Clients must be referred through the St. Joseph’s Infectious Diseases Care Program (IDCP) to be rostered to MyCare. 

Our Impact

Since launching in 2016, MyCare has helped clients start and stay on HIV treatment—many for the first time. The program has demonstrated outstanding outcomes: 

81% of MyCare clients have an undetectable viral load, despite having previously struggled to engage in care 
The team supports clients throughout their health journeys, using intensive outreach and relationship-building to promote trust and adherence.
