We offer primary health care for individuals experiencing homelessness who are not currently connected with a family doctor or nurse practitioner. To receive primary care through our Health Centre, you must become a rostered client. 

If you are not a rostered primary care client and require immediate medical attention, we will direct you to the HOME Busurgent care, or your existing primary care provider if you have one.

To become a Primary Care Client call  519-660-0874 ext. 1290 and ask to make an appointment with a Systems Navigator. 

  • If intake is open, you will be scheduled for an intake appointment. 
  • If intake is closed, you will be referred to other community resources.
