Systems Navigation & Intake Coordination
Learning about all the programs and services at the Health Centre can feel overwhelming. Our Systems Navigators are here to help. They listen to each person’s story, assess their unique situation, and connect them with the supports that best meet their needs.
If you are interested in accessing our programs, a Systems Navigator can coordinate an intake appointment. During this appointment, they will ask questions about your health, income, housing, language, home country, and support network. This helps determine whether you are eligible for our programs and services.
Getting Started
Our Systems Navigators are committed to helping you find the care and resources you need—whether that’s within the Health Centre or in the wider community.
- Internal programs: If you’d like to access our internal programs, we can arrange an intake appointment with you. Please note that while we offer Primary Care, spaces are limited. If you are not eligible, you can contact Health Care Connect to find Primary Care providers in your area.
- Community resources: If you are looking for help with services outside the Health Centre, a Systems Navigator can connect you to external resources. We can arrange an appointment (virtual or in-person) to support you.
