Health Care Connect can help. This program assists Ontarians by connecting whem with family doctor and nurse practitioners who are accepting new patients. By registering online or by phone, you will be matched with a Care Connector who will guide you through the process and help find a provider close to your location.
The H.O.M.E. Program (Health Outreach Mobile Engagement) provides a multi-disciplinary, multi-sectoral mobile response to improve the health outcomes and health equity of highly marginalized individuals in London.
Accès Franco-Santé London (AFSL) is an information hub for the Francophone community in Southwestern Ontario. The organization provides coordination and guidance to navigate health and social services. By reducing language barriers, AFSL promotes equitable access to quality care and enhances the patient experience for French-speaking residents.
The London-Middlesex Youth Wellness Hub provides integrated primary care, community and social services for youth between ages 12 to 25.
